I ask my patients to fill these intake forms, which often gives me very useful information. The forms do not contain questions like cravings/aversions etc., which I ask during the consultation - so clearly, the forms are not meant to be comprehensive or a substitute for case taking. Indeed, I don't even look at the forms before I listen to the patient for 10-15 minutes. I find it practical to keep the forms limited to 2 pages - and in that space - to get the most useful information, especially that which requires some thought and reflection. In the case intake form for children, I ask one or two questions primarily to make parents more conscious of good and bad habits. You can modify and use these forms for your patients.
